About the Role

We are seeking an experienced support worker to join our expanding Independent Living Services Team in Hereford. As a support worker, you will provide bespoke support packages to young people under Local Authority Care, empowering them to advance their aspirations with greater confidence and capability.

Key Responsibilities
  • Provide strengths-based, trauma-informed support to young people, promoting their independence and well-being.
  • Establish and maintain positive relationships with young people, their families, and other stakeholders.
  • Accurately document notes and ensure seamless communication with support staff for effective handovers.
  • Work collaboratively with the team to achieve high-quality support outcomes.
  • Participate in training and development opportunities to enhance your skills and knowledge.
