About the Role:
Randstad Care is seeking a dedicated and compassionate Social Worker to join our team at the Royal Victoria Infirmary (RVI) and Freeman Hospital in Newcastle. As a Social Worker, you will play a vital role in supporting patients during hospital discharge, ensuring that their needs are met and that they receive the necessary care and support.
Key Responsibilities:
- Conduct comprehensive assessments of patients' needs to determine the appropriate level of care and support required post-discharge.
- Collaborate with healthcare professionals, patients, and their families to develop and implement discharge plans.
- Provide support, advice, and advocacy for patients and their families during the discharge process.
- Manage and maintain accurate and up-to-date records of assessments, care plans, and discharge arrangements.
Requirements:
- Qualified Social Worker registered with Social Work England.
- Proven experience in a social work role, ideally within a hospital discharge or similar setting.
- Excellent communication and interpersonal skills, with the ability to work collaboratively with patients, families, and healthcare professionals.
- Knowledge of relevant legislation, policies, and procedures related to hospital discharge and adult social care.
