Job Title: Hospital Discharge Team

We are seeking a dedicated and compassionate Social Worker to join our Hospital Discharge Team at the Royal Victoria Infirmary (RVI) and Freeman Hospital in Newcastle.

About the Role:

This is an exciting opportunity for a Social Worker to work closely with patients, families, and healthcare professionals to ensure that discharge plans are safe, effective, and meet the needs of each individual patient.

Key Responsibilities:
  • Conduct comprehensive assessments of patients' needs to determine the appropriate level of care and support required post-discharge.
  • Collaborate with healthcare professionals, patients, and their families to develop and implement discharge plans.
  • Provide support, advice, and advocacy for patients and their families during the discharge process.
  • Manage and maintain accurate and up-to-date records of assessments, care plans, and discharge arrangements.
Requirements:
  • Qualified Social Worker registered with Social Work England.
  • Proven experience in a social work role, ideally within a hospital discharge or similar setting.
  • Excellent communication and interpersonal skills, with the ability to work collaboratively with patients, families, and healthcare professionals.
  • Knowledge of relevant legislation, policies, and procedures related to hospital discharge and adult social care.
