Job Description: The successful candidate will conduct thorough assessments of individuals with learning disabilities, identifying their needs and support requirements. They will work closely with other professionals, including health, education, and housing, to ensure a coordinated approach. This includes providing guidance and support to individuals and their families, as well as maintaining accurate and up-to-date case records. The ideal candidate will possess a degree in Social Work, registration with Social Work England, and experience working with adults with learning disabilities.

Responsibilities:
  1. Assess individual needs and develop personalized support plans.
  2. Collaborate with professionals from various sectors to ensure effective service delivery.
  3. Provide emotional support and guidance to individuals and their families.
  4. Manage and maintain accurate case records.

Qualifications and Skills:
  • Degree in Social Work
  • Registration with Social Work England
  • Experience working with adults with learning disabilities
  • Excellent communication and interpersonal skills
  • Ability to work under pressure and meet deadlines
Estimated salary: £30 per hour.
