This is a secondment opportunity for months to support the service's needs.

A vacancy has arisen for a Band 5 District Nurse Caseload Holder in the Buckley District Nursing Team.

The successful candidate must hold or be nearing completion of their Community Specialist Practitioner qualification and possess a Registered Nurse Practitioner designation.

Collaboration with Nurse Case Managers based within the District Nursing team is essential for this role.

Speaking Welsh is desirable but not required for this position; both English and Welsh speakers are welcome to apply.

Main Responsibilities:
  • Manage a defined caseload over an hour period, adhering to best practices.
  • Conduct complex needs assessments for CHC, Health and Social Care services, and provide specialist district nursing care.
  • Work independently with other agencies to deliver patient services without direct supervision.
  • Undertake various management tasks and deputise for the Team Leader as needed.
  • Hold a Specialist Practice Qualification in District Nursing or be working towards it, and possess a registered Nurse Prescriber status.
