Our services include IBD nurse clinics, telephone support, biologic infusion services, and inpatient in-reach services. You will work closely with the gastroenterology multi-disciplinary team to deliver specialized nursing care and leadership competencies that contribute to the effective and economical use of resources.
Key Responsibilities- Provide expert nursing care to patients with IBD, including assessment, planning, implementation, and evaluation of care.
- Develop and implement individualized care plans that address the physical, emotional, and social needs of patients.
- Collaborate with the multidisciplinary team to ensure seamless care transitions and optimal patient outcomes.
- Conduct education and training sessions for patients, families, and healthcare professionals on IBD management and treatment options.
- Participate in quality improvement initiatives to enhance patient care and services.
- Registered General Nurse with a degree or degree-level education.
- Experience in IBD nursing or a related field, with a strong understanding of aetiology, pathogenesis, and treatment options.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Ability to work autonomously and as part of a team, with a strong commitment to delivering high-quality patient care.
