**About the Role**

We are looking for a highly motivated and enthusiastic nurse with advanced evidence-based Palliative Care Skills, along with a V300 prescribing qualification and proven advanced communication skills.

**Main Duties**
  • To provide professional leadership, advice and support in palliative care for health professionals, patients and carers to enhance the quality of life for people with life-limiting illness requiring palliative care.
  • Work with multidisciplinary colleagues to provide holistic needs assessment for patients and advice and support for health professionals providing palliative and end-of-life care in the community setting.

The salary for this role is estimated to be around £45,000-£60,000 per annum, depending on experience.
