Care and Community Support Officer

Role Overview: As a Disability Care Assistant at Brothers of Charity Services Lancashire, you will be responsible for providing person-centred care and support to individuals with learning disabilities and vulnerable adults.
About the Job: Your primary role will be to enable and support individuals to make informed choices about their care and support, promoting self-advocacy and access to mainstream services that respect their cultural and spiritual beliefs.
Your Key Responsibilities:
- Supporting individuals with daily living tasks, such as personal care and meal preparation
- Assisting individuals to access mainstream services and activities that promote social inclusion
- Providing emotional support and advocacy to help individuals achieve their goals and aspirations
- Working collaboratively with other healthcare professionals to develop and implement personalized care plans
