About the Role:

We are seeking a skilled and compassionate Care Coordinator to join our team at Cardea Healthcare. As a Care Coordinator, you will play a vital role in supporting individuals within their own homes or in the community, providing respite care to their carers, and increasing social inclusion and life skills.

Main Responsibilities:

  • Support individuals in their daily lives, promoting independence and dignity
  • Assist with employment or education opportunities
  • Manage challenging situations and behaviours
  • Write accurate and factual daily reports
  • Work in a team and on an individual basis
  • Assist with managing budgets and finances
  • Encourage independence in everyday tasks
  • Assist with personal care (dependent on need)

Requirements:

  • At least 6 months care experience
  • Ideal candidate will have a driving licence and access to their own vehicle
  • Up-to-date DBS
  • Willingness to travel

Essential Attributes:

  • Self-motivated, responsible, and able to act on your initiative
  • Good communication and interpersonal skills
