About the Role

We are seeking an experienced and skilled Adult Advanced Practitioner to join our team at Bridge Medical Centre. As a key member of our primary care team, you will play a vital role in delivering high-quality patient care and contributing to the success of our practice.

Key Responsibilities
  • Assess, diagnose, and treat patients presenting with minor illnesses and undifferentiated conditions
  • Triage and treat patients wishing to see a healthcare professional, making necessary referrals to other members of the primary healthcare team
  • Advise patients on general health care and minor ailments, with referrals to other members of the primary and secondary healthcare team as needed
  • Undertake home visits for acute conditions, as assessed by your duty doctor, and within the boundaries of your professional qualifications
  • Enhance your performance through Continuous Professional Development, imparting your knowledge and behaviors to meet the needs of the service
Requirements
  • An excellent standard of written and verbal English
  • Ability to make autonomous decisions
  • Ability to assess, minimize, and manage risks and defuse stressful situations, ability to stay calm
  • Competent use of IT systems, Excel, Word, and Outlook
  • Use of SystmOne
  • An understanding of Chronic Disease Registers and QOF
Experience
  • Experience of working in an urgent care/emergency setting
  • Experience of working in primary care setting
  • Ability to work within a team to provide the best outcome for the patient
  • An ability to use your own judgment, resourcefulness, common sense, and local knowledge to respond to enquiries and requests while adhering to practice limitations
