NELFT NHS Foundation Trust is seeking a highly skilled and dedicated NHS Specialist Nurse to join our Community Neurological Nursing Team.
About the Role:We are looking for a Registered Nurse with a strong background in Neurological conditions, particularly Multiple Sclerosis, Motor Neurone Disease, and Parkinson's disease. The successful candidate will be responsible for developing and implementing care plans for patients with these conditions, working collaboratively with the Community Rehabilitation Service and local care teams.
Key Responsibilities:- Assess and develop care plans for patients with Neurological conditions, ensuring seamless care and a patient-centred approach.
- Work collaboratively with the Community Rehabilitation Service and local care teams to ensure effective risk management and accident/incident reporting.
- Provide telephone advice and consultations to carers and other healthcare professionals on aspects of care and support.
- Ensure effective risk management at team level by assessing and controlling risk, and ensuring risks are added to the Trust register.
- Current Registered Nurse qualification (Part 1 of NMC register).
- Cert, Diploma or First level Degree in Nursing.
- Working towards or be a Non-Medical Prescriber.
