NELFT NHS Foundation Trust is seeking a highly skilled Specialist Homecare Nurse to join their community neurological nursing team. The successful candidate will be responsible for providing high-quality care to individuals with Multiple Sclerosis, Motor Neurone Disease, and Parkinson's disease in the comfort of their own homes.

Key Responsibilities
  • Hold a current Registered Nurse qualification and be registered with the NMC.
  • Cert, Diploma or First level Degree in Nursing.
  • Working towards or be a Non-Medical Prescriber.
  • Develop and implement a plan of care for individuals with neurological conditions, promoting seamless care and a patient-centred approach.
  • Work collaboratively with the Community Rehabilitation Service and local care teams to ensure effective care.
  • Provide telephone advice and consultations to carers and other healthcare professionals on aspects of care and support.
  • Ensure effective risk management by accident/incident reporting, assessing and controlling risk, and adding risks to the Trust register.
