**About the Role**

  • Caseload Management: Manage a caseload of up to 30 individuals with diverse mental health needs, ensuring tailored care plans are developed and implemented.

We are seeking an experienced Mental Health Specialist to join our Adult Community Mental Health Team in Reading. As a key member of the team, you will be responsible for managing a caseload of complex mental health cases, providing holistic care coordination, and working closely with service users, families, and multidisciplinary teams.

The successful candidate will have proven experience in caseload management and will be able to develop and implement tailored care plans that promote recovery and resilience.

We offer a competitive salary of £25.38 - £27 per hour, as well as opportunities for professional growth and development.

Key Responsibilities:

  • Caseload Management: Develop and implement tailored care plans for up to 30 individuals with diverse mental health needs.
  • Care Coordination: Provide holistic care coordination, working closely with service users, families, and multidisciplinary teams.
  • Diverse Cases: Support individuals with varying severities of mental health conditions, offering interventions that promote recovery and resilience.
