About the Opportunity

This is an excellent chance for experienced nurses to take on new challenges and develop their skills in chronic disease management, immunisations, and general practice care.

Key Responsibilities
  • Develop and implement effective chronic disease management plans for patients with asthma, diabetes, hypertension, and COPD.
  • Conduct routine health screenings, including blood pressure monitoring, cervical smears, and cholesterol checks.
  • Administer routine and travel vaccinations, as well as flu immunisations.
  • Provide expert advice on lifestyle, diet, and exercise to promote patient well-being.
  • Audit and review patient documentation to ensure accuracy and completeness.
Requirements and Benefits
  • NMC Registration: Valid PIN with full registration.
  • Experience: Minimum of 6 months in a primary care setting.
  • Skills: Strong clinical knowledge, experience in chronic disease management, immunisations, and general practice care.
