Discharge to Assess Coordinator

About the Role

This is an exciting opportunity to join the Midlands Partnership NHS Foundation Trust as a Bed Hub Allocation Support Specialist. As a key member of the team, you will play a vital role in ensuring the smooth operation of our discharge to assess (D2A) bed based service.

Key Responsibilities

  • To support the tracking of patients' journeys from referral to discharge in a timely manner using in-house CDT IT systems and Excel, ensuring that all time frames are met.
  • To support the Allocation Team in managing bed capacity across all areas to ensure that all capacity is reported and utilized.
  • To work closely with the team to ensure that bed allocation is appropriate and in line with the service criteria.
  • To support the team in managing daily unmet demand, including attending daily meetings to discuss length of stay (LOS) and flow across all acute services with service leads.
  • To develop relationships with all partners and colleagues to support timely updates and flow through discharge to assess beds.

Requirements

  • Education to NVQ Level 2 standard or equivalent level of knowledge and experience.
  • GCSE English grade C or above or equivalent experience.
  • ICT Qualification to the competency level of ECDL/CLAIT etc.
  • Experience of working within a hospital or community setting and an understanding of patient flow.
  • Experience of using computer-based information systems in an office environment.
  • Ability and experience of using IT systems, including Microsoft Office, and accurate and competent keyboard skills.
  • Experience in work planning and prioritizing.

Desirable Skills

  • Standard IT literate with evidenced experience of relevant IT systems/databases.
  • Understanding of patient flow and how it affects the local health economy, both within a hospital and home setting.
