Job Title: Lead Co-ordinator

We are seeking a highly skilled and experienced Lead Co-ordinator to join our team at Net-Worx (2001) Limited.

About the Role:

The successful candidate will provide a coordinated function within the department to support safe and timely discharges. Working closely with partners at the hospital and within the voluntary sector, you will be responsible for ensuring that patients receive the best possible care and support during their discharge process.

Key Responsibilities:
  • Coordinate the discharge process to ensure timely and safe discharge of patients
  • Work closely with hospital staff and voluntary sector partners to ensure seamless communication and collaboration
  • Develop and implement effective discharge plans to meet the needs of patients and their families
  • Monitor and evaluate the discharge process to identify areas for improvement
