Lead Co-ordinator Job Description

We're seeking a skilled and experienced professional to lead our Hospital Discharge Service at the Integrated Discharge Hub (IDH) at Royal Stoke Hospital. As a key member of our team, you'll play a vital role in ensuring safe and timely discharges for our patients.

Key Responsibilities:
  • Lead and manage a small team to deliver exceptional patient care and support.
  • Collaborate with the IDH Manager and wider team to ensure seamless integration and communication.
  • Develop and implement effective monitoring and reporting systems to track patient progress.
  • Identify and lead on patient resources to support individual needs.
  • Provide flexible cover for Co-ordinators to ensure service demand is met.
Requirements:
  • Education to degree level or substantial experience in a related field.
  • Excellent communication and interpersonal skills.
  • Strong understanding of customer service principles and practices.
  • Ability to work under pressure and manage multiple tasks and demands.
  • Idea generation and implementation skills.
  • Motivational leadership and coaching skills to support a team.
