Exciting Opportunity: Community Nurse Specialist (Band 7)

We are pleased to present a fulfilling position within our compassionate organization, Woking & Sam Beare Hospice. As a Community Nurse Specialist, you will play a vital role in our devoted team, delivering palliative and end-of-life support to patients facing life-limiting conditions in Northwest Surrey. Your responsibilities will include autonomously overseeing patient care, ensuring symptom management, providing emotional assistance, and working collaboratively with a multi-disciplinary team.

  • Expert patient care management
  • Development and oversight of care plans
  • Effective symptom management
  • Comprehensive support for patients and their families
  • Continuous monitoring and assessment
  • Upholding high standards of care delivery
  • Building and maintaining relationships

Requirements:

  • Registered Nurse qualification
  • Post-registration educational courses
  • Palliative care certification
  • Degree or master's level education
  • Nurse Prescriber accreditation
  • Leadership and management training
  • Mentorship or teaching certification
  • UK Nursing Registration
  • DBS Clearance
