We are seeking experienced social workers to join our teams in Hertfordshire County Council, working with older adults, individuals with disabilities, and those requiring integrated discharge services. As a social worker in our council, you will be responsible for providing high-quality care and support to vulnerable members of our community.
Main Responsibilities
- Assess and develop care plans for individuals, ensuring their needs are met and their well-being is prioritized.
- Work collaboratively with multidisciplinary teams, including healthcare professionals, to provide comprehensive support services.
- Conduct home visits and assessments to identify the needs of service users and develop effective support plans.
- Provide guidance and support to families and carers, empowering them to make informed decisions about care and support.
- Stay up-to-date with relevant legislation, policies, and procedures, ensuring compliance and best practice in social work.
Requirements
- Qualified and registered social worker with a strong understanding of social work principles and practices.
- Experience working with older adults, individuals with disabilities, or in integrated discharge services.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with service users, families, and colleagues.
- Ability to work autonomously and as part of a team, prioritizing tasks and managing workload effectively.
