Sanctuary Personnel Ltd is seeking an experienced Adult Wellbeing Practitioner to join our Integrated Neighbourhood Team in Bury, Greater Manchester. This permanent full-time position offers a unique opportunity to work closely with vulnerable adults, assessing their needs, and providing consistent support and monitoring their progress.

About the Role

You will be responsible for conducting comprehensive assessments of adults' needs within the integrated neighbourhood, managing a caseload of individuals, and collaborating with other health and social care professionals to coordinate and deliver integrated care services. You will also identify and connect individuals with community resources, including housing, financial support, and recreational activities, while ensuring their safety and well-being.

Main Responsibilities:
  • Assessment and Support Planning: Conduct comprehensive assessments of adults' needs within the integrated neighbourhood.
  • Case Management: Manage a caseload of individuals, providing consistent support and monitoring their progress.
  • Coordination of Services: Collaborate with other health and social care professionals, including medical teams, community organisations, and voluntary agencies, to coordinate and deliver integrated care services.
  • Community Resource Utilisation: Identify and connect individuals with appropriate community resources, including housing, financial support, and recreational activities.
  • Safeguarding: Identify and respond to safeguarding concerns in accordance with statutory guidelines and local policies.

Requirements

To be successful in this role, you must possess a recognised social work qualification, be registered with Social Work England, and have a comprehensive understanding of current safeguarding policies and procedures. Additionally, you should be capable of actively contributing to the creation and implementation of team and service plans, and able to liaise effectively with individuals such as families, carers, and multidisciplinary team members.
