Job Title: Health Visitor
Bolton NHS Foundation Trust is seeking a skilled and dedicated Health Visitor to join our team. As a Specialist Community Public Health Nurse, you will be responsible for delivering high-quality care to children and families in the community.
Key Responsibilities:
- Deliver the Healthy Child Programme, providing health and development reviews, health promotion, and support for parents, carers, children, and young people.
- Manage a universal plus and partnership plus caseload, working collaboratively with other agencies to ensure protection and the best health outcomes for children and families.
- Work as an autonomous practitioner in Bolton, leading on the delivery of the Healthy Child Programme and providing support to colleagues.
Requirements:
- Qualified Health Visitor with a strong background in community public health nursing.
- Excellent communication and interpersonal skills, with the ability to work effectively with children, families, and other healthcare professionals.
- Ability to work independently and as part of a team, with a strong focus on delivering high-quality care and improving health outcomes.
