Job Summary:
We are seeking a highly skilled and experienced Community Health Specialist to join our team at Dorset Clinical Commissioning Group. The successful candidate will be responsible for providing high-quality person-centered care to individuals with learning disabilities, promoting health and wellbeing, and improving health inequalities.
Key Responsibilities:
- Develop and implement a plan to increase screening uptake, reflecting variations across Dorset, and inform the development of a sustainable delivery model.
- Provide supervision, mentorship, training, and clinical support to students and trained practitioners.
- Work with CTLD MDTs and Primary Care to support access to national screening programs for patients with learning disabilities.
- Collaborate with stakeholders to improve the quality and completeness of Learning Disability registers and prompt review where necessary.
- Undertake a scoping review to identify learnings and areas for improvement.
Requirements:
- Registered practitioner, public health professional, or lived experience peer worker to degree/diploma level supplemented by post.
- Registration diploma level specialist training and/or short courses or demonstrable extensive experience in the relevant specialty.
- Experience at Practitioner Band 5 level or equivalent if non-NHS background.
- Demonstrable knowledge of assessment and health promotion interventions in the area of specialism.
- Ability to prioritize and organize workload effectively.
Desirable Criteria:
- Learning and Assessing in Practice Qualification or equivalent practice assessors training.
- Experience of developing specialist programs of care for individuals or groups of patients/clients and providing highly specialist advice.
- Able to demonstrate specialist clinical reasoning skills to assimilate information and make clinical judgments.
Personal Qualities:
- Able to communicate effectively at different levels of the organization and with staff, patient/service users, visitors, or external organizations.
- Able to overcome barriers to understanding where there are physical or cognitive barriers.
- Experience of planning and organizing complex activities, such as organizing own time and that of junior staff and learners.
- Ability to evaluate care leading to improvement in quality standards and service improvement.
