Job Summary

We are seeking a highly skilled Registered Nurse to join our Hospice team as a Nurse Care Coordinator. This role involves providing comprehensive nursing care to terminally ill patients, supporting their families, and collaborating with the interdisciplinary team to ensure seamless care.

Key Responsibilities:

  • Conduct regular assessments and develop individualized care plans for patients.
  • Deliver high-quality nursing services, including pain management, wound care, and medication administration.
  • Collaborate with other healthcare professionals to coordinate patient care.
  • Foster strong relationships with patients and families to provide emotional support and education.
  • Maintain accurate records and adhere to regulatory requirements.

Requirements:

  • Degree from an accredited school of nursing.
  • Current RN licensure in the state.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and prioritize tasks efficiently.

Estimated Salary: $65,000 - $85,000 per year, depending on experience.
