As a Care Coordinator, you will be responsible for supporting a team of care staff who deliver care in community, ensuring that care and support is delivered in a professional manner and that all documentation is accurate and kept up to date.

Key Responsibilities

  • Organise the staff team, and support them with training and development needs
  • Drive improvements in care delivery by observing, mentoring and upskilling carers
  • Carry out competency assessments and care audits
  • Maintain electronic records such as Care plans, Risk Assessments and Client Reviews
  • Complete medication assessments
  • Work with the rota organiser to ensure that the care calls are scheduled in an effective manner
  • Have a good understanding of clients' needs so that you can respond to changes responsively
  • Liaise with stakeholders such as brokerage teams, healthcare professionals and clients families

About You

We are looking for someone who has experience within domiciliary care in a leadership position such as Field Care Supervisor, Care Coordinator, Care Manager, Care Team Leader or similar. You will have great organisation skills and be able to work flexibly to meet the changing needs of the service. You will have QCF level 3 or above in health and social care and demonstrate a caring and respectful attitude and be committed to personal growth and development.
