We are seeking an experienced Care Manager to join our team at Active Care Group Recruitment. As a Care Manager, you will be responsible for delivering high-quality care to clients in their own homes, working closely with our support workers, clinicians, and management team.
Key Responsibilities:- Have ownership of care packages in your area, ensuring that clients receive the best possible care and support.
- Work with the Business Development Manager to review suitable referrals and produce assessment reports for care packages.
- Build excellent working relationships with clients, families, and external bodies, ensuring that we provide client-focused high-quality care at all times.
- Support with recruitment and training for Support Workers for care packages in your area, ensuring that our team is equipped to deliver exceptional care.
- Ensure all records are accurate and completed in a timely manner, maintaining the highest standards of care and administration.
- NVQ 4 or above in Health and Social Care.
- Full clean driving licence with access to own car, as regular travel to visit clients is required.
- A passion for delivering high-quality client-focused care, with experience in care at Team Leader level or above.
- Previous management experience would be an advantage, but not essential.
- Excellent verbal and written communication skills, with a positive attitude and a commitment to continuous improvement and quality.
- Salary up to £30,000 per annum pro rata, including on-call rota.
