Unity Healthcare Recruitment is seeking a highly skilled and motivated Nurse Practitioner - Community Care to join our team. This role involves working collaboratively within a multidisciplinary team to provide a range of nursing interventions and support the holistic needs of patients with acute, chronic, or complex health conditions. The ideal candidate will have strong clinical skills, excellent communication, and a passion for delivering patient-centred care in a community setting.

Key Responsibilities:

  • Clinical Care
    • Deliver a range of nursing interventions, including wound care, medication administration, catheter care, and end-of-life care.
    • Conduct holistic assessments of patients' physical, emotional, and social needs to develop and implement individualised care plans.
    • Monitor and evaluate patients progress, adapting care plans as necessary to achieve the best outcomes.
    • Provide health education and advice to patients and carers to support self-management and promote independence.
  • Community-Based Care
    • Work autonomously in patients homes and community settings, ensuring safe and effective care delivery.
    • Manage a daily caseload, prioritising tasks and allocating time effectively to meet the needs of patients.
    • Develop and maintain strong relationships with patients, carers, and other healthcare professionals to ensure seamless care coordination.

About the Opportunity:

The estimated salary for this role is £35,000 - £42,000 per annum, depending on experience. This is a full-time position, working 37.5 hours per week. We offer a competitive benefits package, including pension scheme, life insurance, and access to ongoing training and development opportunities.

What We Are Looking For:

We are looking for a highly motivated and experienced nurse who is passionate about delivering high-quality community care. You will be responsible for working independently in patients homes and community settings, managing a daily caseload and developing and maintaining strong relationships with patients, carers, and other healthcare professionals.

Requirements:

  • RGN/RMN qualified and registered with NMC
  • At least 2 years post-qualifying experience in community care
  • Strong clinical skills and knowledge of community care practices
  • Excellent communication and interpersonal skills
  • Able to work autonomously and manage a daily caseload
  • Able to develop and maintain strong relationships with patients, carers, and other healthcare professionals
