An exciting opportunity has arisen to take on a challenging role as a lead nurse within the Continuing Healthcare (CHC) team. As a skilled and experienced registered nurse, you will be responsible for leading assessments and reviews of patients with complex needs.

The ideal candidate will have knowledge of the CHC legislation and process, as well as experience working in the physical disability field. They will be able to demonstrate tact, diplomacy, and effective communication skills when liaising with colleagues, healthcare professionals, and service users' carers and family members.

Main Duties

Key responsibilities include undertaking CHC/Funded Nursing Care (FNC) assessments and reviews, developing and maintaining effective communication networks with health and social care professionals, and working collaboratively with relevant health professionals and support staff to deliver a seamless service.

You will also be required to exercise professional judgement when analysing and communicating complex information, interpret national and regional guidance, and make informed decisions regarding multi-disciplinary team CHC assessments.

In addition, you will support Community Matrons/District Nurses in their case management roles for complex cases, deliver against organisational objectives, and prioritise your workload to meet tight deadlines.

Salary and Benefits

The salary for this role is £55,000 - £60,000 per annum, depending on experience. We offer a range of benefits, including flexible working arrangements, access to training and development opportunities, and a comprehensive pension scheme.

Job Responsibilities
  1. Undertake CHC/FNC assessments and reviews for patients across all aspects of care.
