About the Role

We are seeking an experienced and skilled Advanced Practitioner to join our team at Central and North West London NHS Foundation Trust. As a key member of our healthcare team, you will be responsible for delivering high-quality patient-centered care, promoting clinical excellence, and providing clinical advice and support to nursing and AHP staff and other healthcare professionals.

Key Responsibilities
  • Perform clinical procedures, including further invasive testing and treatments, as indicated by the patient's history and clinical examination.
  • Request investigations, such as blood tests, electrocardiograms, and ultrasound scans, in accordance with IR(ME)R regulations and local policy.
  • Prioritize health problems and intervene appropriately in complex, deteriorating, urgent, and emergency situations, including initiation and leadership of resuscitation.
  • Work as an autonomous professional, within ethical codes and legal frameworks, being responsible and accountable for decisions, actions, and omissions at this level of practice.
  • Advocate high-quality care for people, act as a champion for individuals, and represent their interests at team meetings, during hospital care, home care, and when interfacing with statutory and non-statutory service providers.
  • Influence, support, and take an active role in Public Health and Self-care of the individual and community.
  • Collect, collate, evaluate, and report clinical information, maintain accurate patient records related to assessment and care planning.
  • Develop clinical protocols and guidelines within own area of practice to ensure quality of care at all times.
Requirements
  • MSC or PGdip in Advanced Clinical Practice or equivalency criteria as per national standards.
  • Current registration with NMC, HPC, and/or other statutory body.
  • Specialist Practitioner.
Desirable
  • Relevant Professional health degree.
  • Evidence of recent CPD.
  • Teaching qualification.
  • Additional Leadership training.
  • V300 (dependent on role).
  • Appropriate Therapy Training/qualification.
Previous Experience
  • Substantial post-registration experience.
  • Leadership experience at Band 7 or above.
  • Proven experience of working as an independent practitioner.
  • In-depth knowledge of recent NHS legislation, relevant to community.
  • Experience of leading, planning, and completing audit/research.
  • Experience of teaching and mentoring of staff across the multi-disciplinary team.
  • Extensive clinical experience.
  • Involvement in advanced practice development.
