Central and North West London NHS Foundation Trust is seeking a highly skilled and experienced Registered Adult Nurse to fill the Advanced Practitioner Hospital Services position. The ideal candidate will have a strong background in clinical practice, leadership, and education, with a proven track record of delivering high-quality patient-centered care.

Key Responsibilities
  • Practice at an advanced level, demonstrating in-depth knowledge and competence in aspects such as acute medicine, community services, and mental health.
  • Deliver high-quality patient-centered care, prioritizing health problems and intervening appropriately in complex, deteriorating, urgent, and emergency situations.
  • Work as an autonomous professional, within ethical codes and legal frameworks, being responsible and accountable for their decisions, actions, and omissions.
  • Advocate high-quality care for people, act as a champion for people by representing/supporting the persons interests at team meetings, during hospital care, home care, and when interfacing with statutory and non-statutory service providers.
  • Collect, collate, evaluate, and report clinical information, maintain accurate patient records related to assessment and care planning.
Requirements
  • MSC or PGdip in Advanced Clinical Practice or equivalency criteria as per national standards.
  • Current registration with NMC, HPC, and/or other statutory body.
  • Specialist Practitioner.
  • Substantial post-registration experience.
  • Leadership experience at Band 7 or above.
  • Proven experience of working as an independent practitioner.
