Job Description

This role involves developing and maintaining person-centered support plans, ensuring compliance with legal and safety standards. You will also support individuals in financial planning, medication self-administration, and regular health reviews.

Responsibilities
  1. Develop and implement detailed support plans tailored to individual needs.
  2. Provide ongoing support and guidance to individuals, including financial planning and medication management.
  3. Collaborate with colleagues to ensure effective delivery of care services.
  4. Contribute to the development of policies and procedures that promote best practice in care delivery.

Salary Range: £35,000 - £45,000 per annum.
