Community Nursing Team Lead

**Job Summary**
We are looking for a skilled and dedicated Community Nursing Team Lead to join our team at Kent Community Health NHS Foundation Trust.
The successful candidate will be responsible for leading a team of nurses to provide high-quality care to patients in their own homes.
The ideal candidate will have strong leadership skills, excellent communication skills, and the ability to work collaboratively with other healthcare professionals.
Key Responsibilities:
- To lead and manage the Community Nursing Team to ensure high-quality patient care
- To develop and implement new initiatives to improve patient outcomes and experience
- To provide support and guidance to nursing staff on end-of-life care
Requirements:
- RN or RGN with relevant post-graduate qualifications and experience
- Strong leadership and communication skills
- Ability to work collaboratively with other healthcare professionals
Salary: £45,539 - £56,394 per annum
