Case Manager

CAMBA Inc is seeking a skilled Case Manager to join our Healthy Families Program team. As a Case Manager, you will provide essential support to families in need, helping them navigate the challenges of pregnancy and parenthood.
Key Responsibilities:- Assess client needs and develop personalized service plans to address their goals and challenges.
- Provide referrals to community resources and services to support client well-being.
- Establish trusting relationships with clients and their families, promoting healthy parent-child interactions and overall family support.
- Collaborate with clients to identify and address financial, emotional, and other stressors that may impact their well-being.
- Conduct outreach and engagement activities to recruit participants and promote program services.
- Prepare and revise service plans, including short-term and long-term client goals, in collaboration with clients.
- Assist clients in accessing community resources and services, both within and outside CAMBA.
- Experience working with high-needs/vulnerable children and families in a community setting.
- Ability to work with diverse populations and adapt to changing program needs.
- Knowledge of infant and child development, with a focus on nurturing and supportive relationships.
- Strong communication and interpersonal skills, with a ability to build trust with clients and their families.
- Flexibility to work some evenings and weekends, as needed.
- A competitive salary range of $35,000-$38,000 annually.
