Case Manager

CAMBA Inc is seeking a skilled Case Manager to join our Healthy Families Program team. As a Case Manager, you will provide support to individuals and families in need, assessing their needs and developing personalized service plans to help them achieve their goals.
Key Responsibilities:- Provide social service assistance to individuals and families, including referrals to community resources and support with goal setting and achievement.
- Assess applicants for services and develop service plans that address their unique needs and goals.
- Establish trusting relationships with families and provide guidance on parenting and child development.
- Collaborate with participants to identify and address financial or emotional stress, as well as other family risk factors.
- Conduct outreach and recruitment efforts to engage with the community and promote program services.
- Prepare and revise service plans, including short-term and long-term participant goals, in collaboration with participants.
- Assist participants in accessing community resources and services, both within and outside of CAMBA.
- Experience working with high-needs/vulnerable children and families in a community setting.
- Ability to work with diverse populations and establish trusting relationships.
- Knowledge of infant and child development, with a focus on nurturing and support.
- Open to reflective practice and supervision.
- Ability to work independently and as part of a team.
- Competitive salary range: $35,000-$38,000 annually.
