About the Role

The Advanced Practitioner will work autonomously in the Acute Service within the well-established, dynamic and progressive Medicine for Older Adults team, covering seven days including public holidays. They will be responsible for independent clinical decision-making for a complex population including assessment and treatments.

Key Responsibilities
  • Provide advanced clinical assessment, clinical care and management of both stable and acutely unwell patients, making complex, autonomous decisions using advanced skills and knowledge.
  • Have overall responsibility for the assessment of care needs and the delivery of advanced care.
  • Work closely with the multidisciplinary team (MDT) and families to ensure person-centred care with excellent communication skills underpinning this role.
Requirements
  • Registered Nurse / AHP / Pharmacist with valid NMC/HCPC / GPHC Registration.
  • Educated to/working towards/operating at Masters Level.
  • Post Graduate Diploma in Advanced clinical practice and expertise within a speciality/area of practice that demonstrates the required breadth of knowledge required to lead safety, effectively and efficiently.
