We are looking for a highly skilled Senior Healthcare Coordinator to join our team in Tunbridge Wells, Kent. As a key member of our team, you will be responsible for delivering high-quality care services to elderly individuals, people with disabilities, and those requiring palliative care.

The successful candidate will have previous experience in care coordination, management, or a supervisory role in a domiciliary care setting. They will also be familiar with care standards and regulatory requirements, such as CQC guidelines and care planning processes.

Job Description:

  • Develop and implement personalized care plans for service users
  • Ensure that care is delivered in line with each individual's care plan, maintaining a high standard of care
  • Coordinate care staff schedules to ensure service coverage for all shifts and care requirements
  • Maintain effective communication with service users, their families, and healthcare professionals to ensure continuity and quality of care

Requirements:

  • Estimated salary: £32,000 - £45,000 per annum, depending on experience
  • Previous experience in care coordination, management, or a supervisory role in a domiciliary care setting
  • Experience in elderly care, palliative care, or care for individuals with disabilities
