Job Title: Clinical Pharmacist

Job Summary:

The Windrush Medical Practice is seeking a highly skilled Clinical Pharmacist to join our team. As a Clinical Pharmacist, you will play a vital role in providing expert medication management and patient care services to our patients.

Key Responsibilities:

  • Provide medication management services to patients, including medication reviews and reconciliation.
  • Collaborate with healthcare teams to ensure safe and effective medication use.
  • Develop and implement medication therapy plans to improve patient outcomes.
  • Conduct medication education and counseling sessions for patients and caregivers.
  • Participate in quality improvement initiatives to enhance patient care and safety.

Requirements:

  • BSc/Grad Dip in Pharmacy Practice.
  • A minimum of one year's post-qualification experience.
  • Membership as a practicing pharmacist with The General Pharmaceutical Council.
  • Commitment towards continuing professional development in accordance with requirements of The General Pharmaceutical Council.
  • Pharmacist independent prescriber registered with The General Pharmaceutical Council.
  • Relevant post-registration experience of community, hospital, and/or primary care pharmacy practice.

Desirable Qualifications and Skills:

  • Independent Prescriber.
  • Good interpersonal skills with the ability to communicate effectively with a diverse range of people.
  • Ability to work as part of a team and autonomously.
  • Knowledge of IT systems, email, word, and Microsoft Teams.
  • Excellent written and verbal communication.
  • Good time management skills, able to prioritize and organize workload for best effect.
  • Ability to promote best practice regarding prescribing issues.
  • Ability to maintain a high level of confidentiality and discretion at all times.
  • Problem-solving and analytical skills.
  • Ability to follow policy and procedure.

Personal Qualities:

  • Sensitive and empathetic in distressing situations.
  • Ability to work under pressure.
  • Flexibility to work outside of core office hours if required.
  • Clear vision of role and commitment to working in Primary Care.
  • Disclosure Barring Service (DBS) check.
