This is an excellent opportunity for a qualified Pharmacist wishing tojoin part of the Primary Care Team, under the Additional Roles ReimbursementScheme.
We have a rapidly expanding team of healthcare professionals within thePractice, including GPs, Nurses, Health Care Assistants, Care-Coordinators andClinical Pharmacists. We are also a training practice for GP Registrars andregularly have medical students with us from both the University of Oxford andUniversity of Buckingham.
The PCN Clinical Pharmacist is an integral part of the clinical team,using and sharing pharmacy expertise to support the GPs and the whole practicein providing optimal patient care.

Job Purpose
The key aims for the clinical pharmacist are:
Increase safety and quality of prescribedmedicines
Provide support on medicines related issues tothe whole practice team
Be a point of contact for patients regardingmedicines queries

Responsibilities underpinningthe role
The following are the coreresponsibilities of the Clinical Pharmacist. On occasion there may be arequirement to carry out other tasks: this will be dependent on factors such asworkload and staffing levels:
Todiscuss medication issues with patients as needed and appropriate by telephone, adverse effects/interactions, overdose/inadvertentingestion, OTC remedies, queries from dispensary, queries from care homes.
Torespond to dispensary queries to include, re-authorise repeats where there arequeries, clarify doses, clarify products, give appropriate alternatives when availabilityissues.
Todiscuss specific patient and prescribing issues ( complicatedregimes/polypharmacy, compliance difficulties, multiple adverse effects,medication reduction regimes) with other clinicians as necessary.
Tosupport the completion of pink (medication administration) forms for districtnurses for GPs to sign.
Tovisit nursing homes to carry out reviews of MAR sheets/patients medications.
Medicines Reconciliation :
To review secondary care requests for newmedication (as communicated by discharge summaries, outpatient letters, etc.) andraise any queries with relevant GP, and discuss how to respond wheninappropriate prescribing requests arise.
To deal with anticoagulant start/stop requestsfrom secondary care and contact patients who have defaulted on INRs.
MedicinesInformation/Education:
To monitor and inform colleagues as relevantabout ongoing prescribing issues, new guidelines (national and local), newproducts being asked for by secondary care, manufacturing and supply problems,new prescribing restrictions or contraindications, and individual andsystematic errors made by colleagues.
Toadvise on cost effective prescribing and prescribing budget issues.
Tokeep the prescribing folder on the practice intranet up to date, with adminsupport.
Totrain nursing home staff in medicines management.
Prescribing Systems andPolicies:
To identify patients in need of medication reviewand develop the system of patient invitation with the admin team.
To liaise with dispensary, admin and clinicalstaff in discussing, developing, and implementing medicines management systemsin the practice.
To work with the GPs (especially the prescribinglead), Senior Clinical PCN Pharmacist, practice manager, dispensary manager anddispensary team to review, develop and implement prescribing policies andstrategies for the whole practice.
To suggest and design audits in relation toprescribing targets, implementation of locality policies and the QualityOutcomes Framework (QOF).
To work with the GPs, Senior Clinical PCNPharmacist, practice manager, and dispensary manager on choosing and deliveringtargets for the local Prescribing Incentive Scheme (PIS).
To monitor the practices prescribing performance(via the CCG prescribing dashboard, progress on the PIS, ePACT, etc.), andsuggest changes to practice prescribing as appropriate.
To support the delivery of electronicprescribing.
To work with the dispensary team, Senior ClinicalPCN Pharmacist and lead dispensing GP to maximize profitability and to performaudits.
Liaison with communityand hospital pharmacies:
To support and advise dispensary when needed.
To make best use of community pharmacy medicineuse reviews (MURs) for medication reviews.
To rationalise requests for dossette medications.Dispensary will still be responsible for ringing round re availability of products,taking initial calls, requesting new dosettes etc.

Qualifications
Essential
BSc/Grad Dip in Pharmacy Practice. A minimum of one years post qualification experience. Membership as practising pharmacist with The General Pharmaceutical Council. Commitment towards continuing professional development in accordance with requirements of The General Pharmaceutical Council. Pharmacist independent prescriber registered with The General Pharmaceutical Council. Relevant post registration experience of community, hospital and / or primary care pharmacy practice.
