Job Title: Senior Care Manager

Job Summary:

Voyage Care is seeking a Senior Care Manager to lead our supported living services in Swindon. As a Senior Care Manager, you will be responsible for managing a team of colleagues and helping people gain more independence in their daily lives.

Key Responsibilities:

  • Develop and implement person-centred support plans
  • Conduct care needs assessments
  • Induct new colleagues
  • Complete supervisions and appraisals
  • Complete medication competencies and audits
  • Attend multi-disciplinary meetings
  • Complete reports

Requirements:

  • Previous experience in a care management role
  • Level 3 in Health & Social Care
  • Experience working with individuals with autism, acquired brain injury, complex mental health needs, traumatic backgrounds, and challenging behaviours
