Job Description

We are seeking an experienced pharmacist to join our team at Oxford University Hospitals NHS Foundation Trust. As a Pharmacist for Community Care, you will play a vital role in providing high-quality patient care and contributing to the success of our Best Value Medicines and Biologics Team.

The ideal candidate will have a strong background in direct patient care, with experience in managing homecare prescriptions and optimising medicines use. You will be responsible for delivering compassionate excellence by providing a patient-focused clinical pharmacy service, communicating effectively with healthcare professionals and patients, and participating in antimicrobial stewardship initiatives.

In this role, you will have the opportunity to work closely with GPs, community pharmacy teams, and homecare providers to ensure continuity of care during biologic therapy switches. You will also participate in quality improvement initiatives and contribute to the development of new policies and procedures.

Key Responsibilities:

  • Deliver high-quality patient care through effective management of homecare prescriptions and optimisation of medicines use.
  • Communicate effectively with healthcare professionals and patients to facilitate shared decision-making.
  • Participate in antimicrobial stewardship initiatives and contribute to the development of new policies and procedures.

Requirements:

  • Masters Degree in Pharmacy & GPHC registration.
  • DIRECT PATIENT CARE experience in more than one setting or clinical area.
  • Post Graduate Certificate in Pharmacy Practice and due to complete the diploma, or equivalent external credentialing via CPPE/ RPS.

Estimated Salary: £45,000 - £60,000 per annum, depending on experience.
