About the Role

We are seeking an experienced Social Worker to join our dynamic Adult Community team, dedicated to providing support and intervention for individuals aged 18 and above, living with physical disabilities, chronic conditions, autism, and sensory impairments.

As a crucial member of our team, you will collaborate with individuals, families, and caregivers, conduct Care Act 2014 assessments, evaluate needs, and manage both short-term and long-term cases.

Main Responsibilities

  • Undertake complex person-centred assessments with customers and plan for the provision of support and/or services to assist them meet their outcomes and live as independently as possible.
  • Support carers to continue in their role in complex situations.
  • Ensure accurate and timely recording of all information and activity onto the social care records database.
  • Contribute to local knowledge about universally available services and signpost/advocate for customers accordingly.
  • Practice in line with legal requirements, demonstrating a detailed understanding of the Care Act, Mental Capacity Act, and Continuing Health Care guidance.

Requirements

  • Registered with Social Work England.
  • Solid understanding of the Care Act 2014.
  • Ability to approach risks with positivity.
  • Passion for making a difference.
