We are seeking a dedicated Care Coordinator to join our team at Elysium Healthcare in Staffordshire. As a Care Coordinator, you will play a vital role in providing care and support to individuals with Mental Health conditions.

About the Role:

You will be working as part of a supportive and friendly team to provide care and support to service users. Your day-to-day tasks will include engaging with service users, supporting and encouraging them with all aspects of their recovery and treatment goals, such as daily living tasks, community involvement etc.

You will also be required to carry out duties allocated by the Multi-Disciplinary Team (MDT), update patient records, respond to incidents, support mealtimes, and ensure a clean and safe environment.
