The Forward Trust is seeking a Community Health Support Worker to join our Reconnect service in Suffolk. This role involves working closely with colleagues to deliver a high-quality service safely, making referrals to our Meet and Greet service, and ensuring that service users are fully connected with local services before their release into the community.

In this position, you will work with vulnerable adults who require support with their health needs, providing them with the necessary connections to local services to help them build productive lives. The successful candidate will enroll onto an apprenticeship relevant to the role and have experience of supporting vulnerable adults.

As a Community Health Support Worker, you will be responsible for:

  • Working closely with your colleagues to discuss and share best practices to deliver a high-quality service safely
  • Making referrals to the Forward Trust Meet and Greet service
  • Ensuring that service users are fully connected with local services in preparation for release into the community

Our estimated salary for this position is £22,000 - £28,000 per annum, depending on experience.
