About the Role

We are seeking a skilled and experienced Clinical Lead Practitioner to join our team at NHS. As a key member of our Integrated Urgent Care Service, you will play a crucial role in supporting clinical work and acting as a positive role model for junior staff.

Job Summary

  • Proactively identify, diagnose, and manage treatment plans for patients at risk of developing long-term conditions or deteriorating outside of the hospital setting.
  • Triage calls from external clinicians, patients, and others into various services as required.
  • Work with other clinical teams to improve healthcare quality, responding to local and national policies and initiatives.

Key Responsibilities

  1. Recognize and work within your own competence and professional code of conduct, regulated by the NMC/HCPC.
  2. Produce accurate, contemporaneous, and complete records of patient consultations, consistent with legislation, policies, and procedures.
  3. Prioritize, organize, and manage your workload to maintain and promote quality.
  4. Initiate and participate in quality governance systems and processes, including clinical audit, peer review, and quality assessment tools.
  5. Evaluate patient responses to healthcare provision and the effectiveness of care.
  6. Use structured frameworks to manage complaints, incidents, and near-miss events.
  7. Assess the impact of policy implementation on care delivery.

Requirements

  • Masters-level qualifications in subjects related to primary care.
  • Comprehensive knowledge of domestic violence, vulnerable adults, substance abuse, and addictive behavior.
  • Strong leadership qualities and ability to instill these within clinical teams.

Salary: £65,000 - £85,000 per annum (dependent on experience), plus benefits.
