About the Role
As a Clinical Care Specialist at Healthcare Homes, you will be responsible for providing high-quality care to our residents. Your duties will include:
  • Administering medicines and maintaining accurate records
  • Maintaining effective communication with residents, families, and healthcare professionals
  • Supporting new admissions and discharges, ensuring a smooth transition for residents
  • Writing detailed care plans, including risk assessments and diet plans
  • Collaborating with the care team to ensure the highest standard of care
