Job Summary

The Community Children's Nursing Team at Solent NHS Trust is seeking a highly skilled and experienced Senior Integrated Community Children's Nurse to join our dynamic team. As a key member of our team, you will be responsible for providing high-quality, family-centered care to children and young people with complex health needs in their own homes, schools, and communities.

Main Responsibilities
  • Assess and develop individualized care plans for children and young people with complex health needs
  • Provide skilled nursing care, including wound management, IV therapy, and medication administration
  • Work collaboratively with families, schools, and other healthcare professionals to ensure seamless care coordination
  • Supervise and mentor junior staff, providing guidance and support to ensure high-quality care
  • Participate in multidisciplinary team meetings to discuss patient care and develop care plans

Person Specification
  • Registered Nurse with a valid NMC registration
  • Experience in community children's nursing or a related field
  • Strong assessment and planning skills, with the ability to develop and implement individualized care plans
  • Excellent communication and interpersonal skills, with the ability to work effectively with families, schools, and other healthcare professionals
  • Ability to work autonomously, with minimal supervision, and to prioritize tasks and manage time effectively
