About the Role
We are seeking a highly motivated and empathetic individual to join our team as a Social Prescribing Link Worker. As a key member of our Primary Care Network, you will play a vital role in supporting patients with their health and wellbeing needs.
Key Responsibilities
As a Social Prescribing Link Worker, you will work closely with our practice teams to identify patients who would benefit from social prescribing. You will then work with these patients to understand their needs and develop a personalized plan to support them. This may involve connecting them with local community groups, statutory services, or other resources that can help them achieve their goals.
Requirements
To be successful in this role, you will need to have a good understanding of social prescribing and its benefits. You will also need to be able to build strong relationships with patients, practice teams, and other stakeholders. A full driving license and access to a car are essential for this role, as you will be required to travel across the locality to visit patients in their own homes.
