Thames Reach is a leading organization in London that aims to end homelessness. We operate projects and services that directly help homeless people on the streets and in hostels.

We work to prevent vulnerable individuals from becoming homeless and help those who are already homeless to get their lives back on track. Our teams operate across London, providing support and guidance to those in need.

About this Role

The Thames Reach Outreach teams respond rapidly and flexibly to reach people who are rough sleeping. Our staff assess individuals to identify their support needs and create personalized plans to achieve independence.

  • Lead regular outreach shifts with partner agencies or volunteers, building trusting relationships with new clients.
  • Manage a caseload of rough sleepers, devising effective plans to help them overcome their challenges.
  • Improve local responses to rough sleeping by coordinating joint working between agencies.
  • Represent the team at multi-agency meetings and case conferences.
  • Identify gaps in local provision and research new pathways off the streets for rough sleepers, including those with no recourse to public funds.
  • Work across various databases to accurately record activities.

To succeed in this role, you will have:

  • A strong commitment to delivering high-quality services.
  • Experience working with vulnerable adults and supporting them to access essential services.
  • Excellent organizational and time management skills to coordinate multiple cases and complete tasks efficiently.
  • Strong communication skills and confidence in public speaking when representing Thames Reach externally.
  • Ability to form effective working relationships with partner agencies, challenging them when necessary.
  • Proficiency in using Outlook, online recording systems, Excel, and Word, as well as tablet and smartphone technology.
