Job Title: Social Worker

We are seeking a highly motivated and enthusiastic Social Worker to join our team at West Berkshire Council.

About the Role

This is an exciting opportunity to work with vulnerable individuals, promoting their rights and safeguarding those at risk of abuse. As a Social Worker, you will be responsible for assessing and reviewing eligibility for long-term support, working with the Safeguarding Team, and completing S42 enquiries.

Key Responsibilities
  • Support the safe discharge of service users from hospital through a joint Health and Social Care approach.
  • Assess or review eligibility for long-term support in accordance with the Care Act 2014.
  • Work with vulnerable individuals who are unable to make decisions in accordance with the Mental Capacity Act 2005.
  • Promote the rights of individuals and safeguard those at risk of abuse.
