As a Care Coordinator at Cygnet Hospital Bury, you will have the opportunity to make a positive impact on the lives of patients and their families. With over 30 years of experience in providing high-quality specialist mental health services, Cygnet is committed to delivering exceptional care and support.

The successful candidate will be responsible for working in partnership with service users, their families, and other stakeholders to assist with the recovery process. This will involve forging links and maintaining contact with other statutory agencies, leading on the social care needs of service users, and ensuring that they and their families are aware of their legal rights.

You will also take an active role in safeguarding within the multidisciplinary team and provide written reports to the team and other professionals.

Requirements:

  • A Registered Social Worker with Social Worker England
  • A relevant Social Work qualification
  • Experienced in working within a mental health setting or passionate about working in this field
  • Knowledgeable of safeguarding, health & safety, data protection, confidentiality issues & equality & diversity
  • An excellent communicator with exceptional interpersonal skills
  • Able to work as part of a multidisciplinary team
  • Skilled in report writing with excellent literacy skills and attention to detail

About the Role:

This is a full-time position, working 40 hours per week, Monday to Friday. You will be based at Cygnet Hospital Bury, which provides a wide range of inpatient mental health services for men and women, including specialist deaf services and CAMHS provision for adolescents.

The hospital features en-suite bedrooms, communal spaces, including a gym, laundry, occupational therapy kitchen, therapy garden, and woodland walkways down to the Elton Reservoir.

Salary: £28,000 - £32,000 per annum, depending on experience.
