Community Nurse Position

We are seeking a highly motivated and experienced Community Nurse to join our team at Norfolk Community Health and Care NHS Trust. As a Community Nurse, you will be responsible for providing high-quality, patient-centered care to individuals in their own homes, promoting independence and improving health outcomes.
About the RoleThis is an exciting opportunity to work in a dynamic and supportive team, providing care to patients with a range of needs, including those with long-term conditions, palliative care, and wound management. You will work closely with other healthcare professionals, including GPs, social services, and community matrons, to ensure seamless care coordination.
Key Responsibilities- Assess, plan, implement, and evaluate patient care, developing individualized care plans that meet the needs of each patient.
- Provide education, support, and prescribing planned care for individuals and their carers/family with long-term conditions, such as diabetes and wound management.
- Support patients at the most sensitive periods of their life, including palliative care and syringe driver management.
- Work as part of a multidisciplinary team, linking with the local Primary Care Network, wider community stakeholders, and your immediate team.
To be successful in this role, you will need:
